The Sheriff's Office says it happened in the 15-thousand block of U-S 224 east of Kalida.

A car struck a pole in Putnam County on Sunday afternoon.

The Sheriff's Office says it happened in the 15-thousand block of U-S 224 east of Kalida.

The driver was 20 year-old Jade Jennings of Cloverdale.

She was ejected from the vehicle and taken to Saint Rita's Medical Center.

An un-named juvenile passenger was also transported to Saint Rita's.

The accident took place just before 4pm.
